Meniscal fixation is a successful treatment for hypermobile lateral meniscus in soccer players

Purpose:

Report the outcomes of arthroscopic all-inside meniscal fixation in a large sample of soccer players with hypermobile lateral meniscus. A surgical treatment with mainly anamnestic indication.

Symptoms:

  • Lateral-side knee pain with hyperflexion, kneeling or squatting
  • Locking
  • Trauma history?

PE:

  • Meniscal testing not always positive
  • Lateral joint line pain not constant

Methods:

2010-2015: 55 soccer players with hypermobile lateral meniscus treated surgically :

  • 46 cases included.

Retrospective study Phone evaluation (11.2016):

  • Demographic characteristics
  • Tegner
  • VAS
  • IKDC
  • return to play

Surgical treatmenet:

All inside meniscal fixation using mainly two FasT-Fix® devices (Smith and Nephew)

 

Results:

  1. Mean follow-up was 43 months( 8-73 )
  2. Cases of reoperation ( 6,5 % ).
  3. Preop median Tegner score was 9 and 8 in postop.
  4. VAS for pain median postop = 1.
  5. Mean postop IKDC = 86.2 ( SD 16.7 )
  6. Return to play soccer in 38/46 = 82% cases.
  7. Return to the pre-injury activity level = 27/46 = 59% cases

 

Conclusion:

The diagnosis of hypermobile lateral meniscus is a clinical diagnosis only confirmed during the arthroscopy.

Players should be advised that return to preinjury activity level is only 59% of the cases
